COMPREHENSIVE MEDICAL EVALUATION
Our first meeting will be long and in-depth! We will begin with a review of your personal and familial health history, chronic conditions, current health status and your personal goals. From there we will discuss your personalized path to meet that goal, utilizing my unique Five Foundations program. Finally, we will discuss and prescribe the use of pharmaceuticals to aid in your success as is appropriate per patient.
60 Minute Telehealth Appointment $200

After our initial meeting there are two options to guide your path forward:
1) Weekly progress form submission with weekly 15 minute Telehealth appointment ($200/month)
2) Weekly progress form submission with monthly 30 minute Telehealth appointment ($125/month)
